Thank you for submitting an application for Associate Membership in NACHO. A representative from NACHO Operations will reach out to you soon to schedule a Site Qualification Visit (SQV). The NACHO Membership Committee with review new applications for membership twice a year, in July and in January. You must have your application submitted and your SQV completed for your site to be considered.

​Please note that:

• Associate Memberships are contingent upon the execution of a Master Consortium Agreement between your institution and St. Jude Children's Research Hospital in its role as the legal entity and Operations Center for NACHO. 
• Access to the protocol submission packets for individual studies will be provided once the Master Consortium Agreement and Rider are executed and submitted.
